AOS interview for the 3rd time

  • F2B AOS applicant already had initial interview in CA,
  • called for a 2nd interview in CA and found out it was a system fluke.
  • Applicant moved to NY and called for a 3rd interview, the lawyer went to the first two interviews, the 3rd interview applicant went alone. A hurried USCIS adjudication officer said at the interview that there was issues with the filing of the application, so applicant has to wait for a written decision.
How is it possible that the "filing issue" wasn't found in the first interview? the lawyer is pretty sure that the USCIS officer in NY is wrong, and the application was filed on time.

What can be the outcome here? anyone faced similar issues?
