This also happened to us. When we went to the first scheduled appointment we took the letter with us for the second appointment to ask them about it and they just said to ignore it. Something to do with the fact we also applied for AP.

We had an Infopass appointment where they confirmed they had cleared. Also, I might be wrong as it's been a while now, but I seem to recall the date of last update changed on our case status. Although the status text didn't change the date changing indicated something happening on our case.
As far as I know, if it needs signing for then it has to go to the physical address. If Express USPS needs a signature then send it to the physical address.
If you send it regular mail then send it to the PO box (since there is no one to physically sign it at a PO box).

I think it will be fine. Further down the line, if you win, then you have to send in more photos, with stricter guidelines, and they will be the ones that are more important. So at that time you can make sure they are exactly right.

It's not really a big deal. Just enter 0 or NA, it really won't matter, but best to enter something rather than leave blank.

Personally I think '0' is better than 'NA' as '0' answers the question properly.
I paid mine last year with personal check (overlooked the fact it said cashiers) and it was no problem at all. That said, you should do exactly as it says to be on the safe side.
It does say to write the case number on every sheet, but I can not see they would penalise you for not doing so.
The only reason they ask for this is in case your file is accidentally split up or dropped on the floor with other files, or the odd sheet misplaced. Then they can easily see...
