I am thinking it is a personal check that is returned for your record once encashed. I know some banks do that although several banks post copies online. You can always call the bank and see if the check was cashed?

Yes, EAD approval took me just about a month from the Notice Date. However, I don't have the card in my hand yet. I was told it takes about 2 weeks to receive the card in mail.

Getting an RFE is a good sign. Just relax while on vacation and get your documentation ready once you return. There shouldn't be any problems with your re-entry into the US as your AP is still valid. I am guessing your RFE is either related to birth certificates or marriage...
