what travel documents needed to travel on EAD

Guys,
I have EAD and AP,
What other supporting travel documents i need to go to india and come back?

Technically, you don't travel on EAD (EAD states "Not Valid for Reentry") - you travel with AP and re-enter under "parole".

I recently traveled twice abroad (vacation and business) and I was only asked passport and AP (both copies).

Note. I am not an attorney.
 
Is I-94 issued.

When you enter with AP is the I-94 is required to filled up and provided
to the immigration officer?. If so what date would they stamp on I-94.

I-94 (white form) is required. They stamp the date of entry and also put an additional stamp that says validity is for one year (date of entry minus one day).
