What are the reasons of referrals?

If you mean referral to an IJ, it's not just inconsistencies. Unless you had some major inconsistencies in your story which make it clear it's not true, you can usually clarify minor ones with a NOID, a request for evidence/information, or a 2nd interview. Referrals have more to do with the officer not being convinced of your story, the officer not believing you qualify for asylum, or the officer being unable to make a decision bc of legal technicalities that are out of their reach.

As for examples, there are just too many and too subjective to try to enumerate here. It's all up to the officer and their supervisor.

I know inconsistencies. What are the examples of "inconsistencies"?
I recommend watching "Well-founded dear" documentary (2000). It shows the samples of inconsistencies:

1) contradiction between oral and written testimonies.
2) Correcting yourself multiple times over dates, verbs, or details of your story.
