Urgent H1B Extension Question

I got my first H1B approved in April, 2001 with Company A. I got another H1B approved in May, 2001 and my visa was granted till May, 2004. I am working on filing my H1B extension but my HR peope are taking too long to get this started.

My question here is do I have till May, 2004 to file for the extension or April, 2004 since I got my first H1B in April of 2001? Please help.
 
What is the significance of the two approvals you had? Different companies? If May 2004 is the expiry date in the I94 with the current company you have till May 2004 (whatever date is mentioned) to get a H1 Extension.
 
I am sorry but I get really confused when I start naming companies A and B. :) You guessed it right though, the 2nd H1B was from a different company. So you think I have time till the expiry of the current H1B which it does in May, 2004?
