Self Employed This Year - What to put on I-134

I am planning to sponsor my parents for visitor visa.
I started my company in 2006 and have been self-employed (consulting). The company made good money in 2006, 2007 and 2008. But in 2009 due to slowness in job market, I had to take a Full Time job. I drew over 100k through my full time job and also made about 30k revenue on my company.
In October 2009, I quit my full time job and went back to consulting.

On I-134, Affidavit Of Support, What should I show as my status? I believe it's self-employed.

The form state to provide tax return for last year for self-employed. I file company tax (which was low last year) and personal tax. Which one should I show?
My worry is, if I show company tax filing, the amount is not that much. I am self employed, so there is no employer letter.

The form also mentioned that self-employed can provide "report of commercial rating concern".
I tried to look around but could not find where to get this type of report.

Please advice what should I do?
