On May 25, 2004, we mailed a notice requesting additional evidence or
information in this case. The notice explains in detail what additional

I have not received it yet.
I've been changing job (company downsizing) last June, to Company A as as contractor, From Sept to CompanyB as contractor, in May I got offer to convert as regular employee as Company C.
