Re-entry Permit Doubt

Hello,

We are applying for my father-in-laws re-entry permit. Let me first explain his situation. He got his GC in June 2004, My mother-in-law is US Citizen but he does not intend to stay here because his property and family in India so he didnt apply for citizenship. He went to India back in last Sept 2010 and is currently back in US in June 2011. Now we are applying for his reentry permit since he does not want to abandon his GC because of us. Now my Ques is

1) What do we need to put in Part 1 Q3 - Class of Admission?
2) He needs to stay back in India due to his mother's health and might over stay for more than 1 year. Now how do we describe this reason in Part 4. Is there a good format to put in?

Any help is greatly appreciated.
 
Only one form with one document photocopy is required? Just curious and dont want the application to come back. Read through the instructions but still want to make sure once again from other peoples experiences.
 
Yeah, no duplicates needed like with the Indian consulate. Read through the instructions carefully as it has been a while for me, but I'm quite sure you need only one copy of the application, a copy of your GC, and a letter of explanation if you want the application expedited.
