Question about yellow form

I remember when last batch of FP notices arrived around Jan, there were yellow forms accompanied them, asking for employment verification. I didn\'t get the yellow form with my FP notice. Are the same true for other people who received FP notice lately? If so, will the CSC send out a yellow form before the approval?

Just want to be clear of potential harzard.

I received my FP notice in March 2001. I didn\'t get any yellow employment verification form. I completed my FP three months ago. Haven\'t received any yellow forms yet!

The yellow form is sent to NSC folks when they receive
finger print notices. It has nothing to do with us.
Each center conducts things differently. Don\'t worry
about it.
