Medical RFE Question - Please help

I received a Medical RFE for my spouse. Doctor has performed the test but has forgotton to check the appropriate box in the INS-Form.
My Question is , Does the test have to be re-done , or proper completion of the form should be sufficient ?
Any one who has been in similar situation and has successfully replied the Medical RFE , please respond.

Proper completion of form would be sufficent. go to the same place they will have the records
I also got RFE for medical, doctor didn't took chest X-ray.
