MEDICAL FORM problem ??

In my medical form, (section A), there's a blank space for "Intended Canadian Destination".

For mailing address, it simply says "Canada" and not my current address.

For telephone number, it has a blank space.

Rest of the details are all complete and ok.

Is it like this in everyone's medical forms ??
Can someone please tell me if this is ok ??
