I140 approval notice missing

Hi, My employer just notified me that my I140 approval notice has gone missing. I have the receipt number, PD etc... Is it possible to file with just a letter indicating the receipt number and explaining that the approval notice has gone missing and that we have filed for a duplicate... Whom should this letter be from. From my employer or me ...
