I-90 Part3 Questions

I am about to apply for Green card Renewal which was employment based.
I found a few questions in I-90 form look confusing to me but hope you can help me to figure them out.

-Part 3, item1: "Location where you applied for an immigrant visa or adjustment of status:"
Is supposed to be Texas Service Center(SRC-from Green card) or Fairfax, VA (my residence address when applied)?
-Part 3, item2: "Location where your immigrant visa was issued or USCIS office where you were granted adjustment of status:"
Is supposed to be Texas Service Center(SRC-from Green card) or Fairfax, VA (my residence address when issued)?

I looked through some previous forums but the answers looked inconsistent.

Did you figure out what the answer should be? I am in the same boat, trying to get a replacement for my parents’ lost gc. On the back of their card it says “MSC”, we went into the Phoenix AZ field office for interview. So which one should I put down? MSC or Phoenix? Thank you!
