I 693 form question

I printed the latest 693 form from http://www.uscis.gov/i-693

The form has 4 pages (with following written at the bottom)
1)original uscis a-file
2)civil surgeon
3)applicant
4)physician or health department.

Does that mean
1) and 4) need to be send with 485 in sealed envelope
2) doctor will keep
3) i get this copy to see and keep for my record


Also on RHS top corner form says "OMN No. 1615-0033 Expires 06/30/07"
I hope this doesn't mean the given form is expired....

As I have doctor's appointment tomorrow, I would really appreciate experts reply ASAP.

Just show up for your appointment and let the civil surgeon fill out the form, seal the copy (copies) that need to be sealed and give you your copy. That's what you pay them for! Might even provide you with a blank I-693 form.

And don't worry about expiration date.
