Form i-90 GC renewal question

I'm filling out the i-90 form and I have a couple of questions.
In Part 3 this question pops up

  1. Location where you applied for an immigrant visa or adjustment of status:
I lived in Orlando at the time, but my attorney filed for me. He doesn't practice anymore so there's no real way to find out where he sent the forms to. I had my biometrics appointment in Orlando. Would it be ok to put Orlando there?

then this question:

  1. Location where your immigrant visa was issued or USCIS office where you were granted adjustment of status:
I did my interview and was approved in Orlando Florida, but my green card has the MSC (Missouri service center) code on it. Which one should I use?
