Applying for a minor child's second US passport

I am planning to apply for my son's second US passport, the first one is going to expire soon. My son derived US citizenship under CCA when we the parents naturalized in 2008.

I plan to go with him to the post office and submit these:

1) His current passport, and a copy of his current passport,
2) Filled out DS-11,
3) One parent's ID copy,
4) fees.

(He is over 17 but under 18 years old).

Is this list correct?
 
No, I did not get it for him (yet).

Well hurry and submit the application before his passport expires. If submitted after his passport expires they may ask for additional proof of his citizenship (i.e. his citizenship certificate or parent's certificate).

His passport expires in 2 months and I will be applying for a new one this weekend or next.

Is this true? I thought both parents had to sign the DS-11 and provide IDs and ID proofs?
 
Oh interesting, good to know. I am going to be renewing a passport for my son but his application is going to require both parents because he is very young.
