Anyone Applied EAD by self

I am planning to reapply EAD on my own. First EAD lost in mail , more then a month after mail in date, spoke with IIO and am told to apply again. Did anyone do this earlier? What is the process and what documents need to be sent along with the application.

You have to apply like a new EAD(fee, photo etc.), but mention that this is to replace the lost EAD..

If your EAD was approved but lost in the mail, when you reapply, you are legal to work with your receipt notice. Check with your lawyer.
