My suggestion is, you can accept US offer and after getting your pay-stub (at least 3) , then transfer the H1B to your old employer.

You can start working for the new company as soon as they send your papers to USCIS. You woudl get the recipt in 2 weeks or so. If you need to give some notice to old company, give it now and after you get the reciept you may join the new company.
I don't think that is good idea.Is your previous visa still valid? My suggestion is, get stamping in Canada before you go to India. You can get dates in Canada.

You need to surrender only the OLD I-94 that was pinned when you came to US last time. The I-797, you shouln;t surrender as that is you rnew one. You need to get stamping in India and while coming with the papers, at port of entry they will re-issue new I-94.

Yes, you need to go to your Home country to get the stamping if it is first time stamping or it is renewal ( but No need for H1B transfer).
Please check with others too.
The details they asked are related to the employer only. So contact your US employer and get the details and re-submit them. They will mail you these details (Tax details and all) which you shouldn't open it. You submit it, stating additional details as per their request.

You can get an appointment in Delhi or Culcutta ONLY if there is no previous visa rejection. So, better for fresh applicants.

You should contact TTS, but they say we cannot change the Approval details (register number, company details) and also Passport number. They would ask you to cancel the appointment and apply new,BUT contact tts as soon as possible. DONOT CANCEL THE APPOINTMENT by yourself, which may delay your...
You need to get stamping when you come here. You need to get valid multiple entry visa until 2007. But your current visa woudl expire by 2005.
From USA at least , you have to get the stamping done (IF they do now also).
Also check with others.
How did you work without H1B approval? What is the visa status of yours untill Oct 1st? It is illegal, if you show the paystub, that you worked without work permit.
Consult with some lawyers.
