You will definately need a BC or certificate of non availablity of BC along with two affidavites. I got the RFEs for me and my wife. HAd my wife's certificate so used that and used non availability certificate for me with 2 affidavits.

go to nearest INS local office with your birth certificate and original GC with passport and I90. they will send out a new card with correction and will stamp the passport with temp I551 stamp for a year.

AC21 can only be applied with approved 140 and pending 485 for 180 days. There willnot be any case like that. Read AC21 guidelines.

As said earlier in your case you should look for a new H1 ASAP . Since your old employer had send a letter to take back 140 your AC21 letter will hold no value. Ac21 can be applied only if 485 is pending for 6 months with approved 140. If your old employer is not willing to help i guess only...
