The INS may notify you that your 485 case has been transferred to the local office for interview. The AVM message will change and you will get a notification letter.
Of course all this happens before (or instead of) the approval.
Few people are in this situation.

The law does not says that you are supposed to be employed by the company for 180 days. The law says that 180 days after you applied your application is not dependent on your initial I140.
So if you keep your I140 for 180 days and INS is not issuing an RFE you are safe...

I assume that all the days are counted. Otherwise the law would specify 180 work days.

Last time I checked on the revalidation procedure through the DOS office (somewhere on the east coast) one of the conditions was that your original H1 visa should have been expired for less than one year. Let us know if you find more updated information.
