If you don't answer Yes to those tax related questions on N-400, I doubt the IO will ask to see your tax documents. But just bring them in case

Just use correct date and write a note to be attached with copy of GC explaining it. USCIS should have the correct date in their file. Don't spend money on replacement card simply because the date is wrong.

Did you get the GC before or after 1995 DUI arrest? If after that and you already sent original when you did I-485, the USCIS probaly still have the original in their file
