I did a quick research, forms I485, I765 and I131 state that if you are filing concurrently then the address is PO BOX 805887, CHICAGO IL 60680-4120.
Only I-130 says that the address is PO BOX 805887, CHICAGO IL 60680-4107.

Thanks Jackolantern. I am not familiar with I-131. I just read about it. The thing is if I file the whole package right now, will it get approved before December for travel?
They both meant you are allowed to apply (4 years 9 months) 90 days before your 5 year anniversary which is what you did. But you misstated that you applied 180days (4 years and 6 months) before the 5 year anniversary, that if the case application will be denied.
I would be safe and use the date on GC (dates are not too far apart) and apply 90 days before your 3rd or 5th anniversary, depending on your situation.
+1. I setup an Infopass after 30 days and was told that my application is sitting on supervisors desk for final approval.
