Second J1 visa after finishing 3 year J1 visa waiver in USA

Hi Guys,
I would appreciate if someone can share their input on my query :
I am physician from India who did 3 year medicine residency training on J1 visa and then completed 3 year j1 visa waiver requirement( finished last year) by working in an underserved area. I don't have a green card yet ( priority date 2012), I am working on h1b visa now and has got a J1 visa 3 year fellowship position ( with 2 year home residency requirement). My questions are:
1) Can I go back again on J1 visa for this training?
2) would I have to do 3 year J1 waiver again once I finish my training?
3) If yes to question 1, then would my case have any additional difficulty when I go back to India for visa stamping at the time of interview since I did not return to home country last time and did j1 waiver in USA?
4) Can I change my status from H1b to J1 visa while being in USA ( starting training on July 1st 2015). Do I have enough time.

You can switch to J-1, but you have to apply for another J-1 waiver.
You do not need to wait to the end of your 3 year fellowship position, you can apply for J-1 waiver any time during the 3 year training.
