Re-Entry Permit (2nd Time)

Hi all,

My first reentry permit is expired and i am in States at the moment, I would like to reapply my second reentry permit.

What are the documents do i need to attach with the form?

Form G-1145
Form I-131
Filing fee Receipt $445
2 Photographs

Do i need at attached copy my old reentry permit ?
Do i need at attached copy of my green card?
What else am i suppose to attach ?

I never had encontered a problem with a reentry permit (I have had 2) ....things were very smooth.....

What is/are your reasons for getting a re-entry permit? Im a bit worried because my mom spoke to an immigration lawyer and he said having a RP does not guarantee you readdmission.
