Name mismatch in the Passport Biography page and US Visa

Hi,

I am currently living in US on H4 Visa as a dependant of my husband. Both of our Visas expire on 30th of September 2009. His company has started the initial process for his H1 extension and my H4 extension. I have some kind of name mismatch between my currently stamped H4 VISA and the name appearing on the passport biography page.

I'll explain the situation with an example.

For an instance, assume my full name as Anne Mary Frida Jane Joseph (I have 5 names).

  • My passport contains the above given name in the same order.
  • However, the name appears on my H4 Visa is Frida Jane Joseph, which is only 3 names in the same order (which is a subset of the full name).
  • My I-94 contains Anne Joseph.

In all of the above cases Joseph represents my Last Name.

Concern I have is when I am filling I-539, which name should I use. Should I fill as the name appears on the Passport biography page, which is Anne Mary Frida Jane Joseph
- Or -
Frida Jane Joseph, which is in the current H4 Visa.

Note: I received my 1st Visa stamping at the consulate of my home country.

Appreciate if you could shed some light on to this as this is troubling me a lot.
