L1 to H4

My wife currently has L1 visa and want to convert to H4 visa.

Her employer had filed Green card but it has NOT got the Labor approval or cleared the Perm stage.

I have the following questions
1. In DS-156 for question 36 does she need to mention YES or NO
2. Will they ask anythink about current L1 visa or current employment.

A1: As long as an I-140 has not been filed, the asnwer should be no.
A2: They might. Is that something to worry about?
