If your H1 gets approved while you are in India then you can get H1 visa stamp and can come to US on H1 status. If you come to US on H4 status then your employer can file H4 to H1 Change of status in US and can start working once H1 gets approved. FYI at this time many Desi employers are not going to file H1 petitions with USCIS.
