From the instructions, it says type "N/A" if an item is not applicable or the answer is none, unless otherwise indicated. Instructions also says keep all information within the area provided.
For the part 11, a few questions after you answer No, the immediate following questions, will no longer apply to you, neither "yes", nor "no", such as question 18, should I mark a N/A next to "yes" & "no", or just leave the questions not selected without any marks?
Also, for the addresses I lived for last 5 years, if I only had one address to fill out, should I leave the rest provided spaces blank or mark N/A?
