affidavit of support form question

hi every body
what is the right affidavit of support form for immigrants is it i-134 form or there is another one else ? should we list all the family members in one form or every member should be in saperate form ?

Yes its the I-134.
You should list all the Beneficiaries.
And also your sponsor must provide you with the supporting documents, like the W2 last tax report, pay slip....

Absolutely correct - and you only need one I134 to cover all the applicants.
