H4 to H1 Processing

The questions I have are:

1. H4 to H1-B processing - Do we need to file both I-129 and I-539 forms ?? this is to get my wife onto working grounds.

2. Does my wife need to go to the Country where she holds the current passport from in order to get the H1 visa stamped ? or is there a service center with in USA where this can be handled ?

3. Can she start working right after she gets the I-797 (Notice of Approval) or does she have to get a visa ??

I'd really appreciate a reply to these above posts.

1. Need to file I-129.
2. Tricky question, depends on whether H-4 is considered to be in the same category as H-1, there is no guidance.
3. She can.
