J-1 waiver with Indian consulate (chicago) issued passport

Hi,
Thanks for taking the time to answer my questions.
I shall be applying for NORI from the chicago Indian consulate for my J-1 waiver. I came to the US with an Indian passport issued by the regional passport office, Chennai. My passport has now expired and I have obtained a new passport from the US consulate, Chicago. In this scenario, would I still need to send one of the NORI forms to the Chennai regional passport office?
Please advise.
Thanks again.
Sincerely
Sundar R Selvaraj
