My mom sent a FIOA request for my file I -130 petition, the one she petitioned to me.
I signed the form G-639 to give my mom permission to request a copy of her I-130 petition to me.
yesterday my mom received a letter from USCIS-FIOA asking my mom for the:-
DATE OF ENTRY
PORT OF ENTRY

Otherwise they will not give her a copy. my question do they want the date of entry and port of entry for me or for my mom?
and which one is it the first or the last one?

If I were you I would just give both and clarify what date and port of entry is for who. Usually they refer to the last and not the first, but again, I would just list them all. That way, they will not come back saying we meant you and not your mum or the other way round.
