Simple quest. on completing I-131 form

Hi..

I ahev following simple questions to fill I-131 form, I am coming to States just to file this form and will come back again for finger printing, so I just want to make sure from the ppl. who has already sent their forms successfully to answer my simple questions:

1. In the form instructions document it was mentioned to send a front and back copy of your Green card/Permanent Resident card (Form I-551).
IS THIS THE ONLY DOCUMENT THAT WE NEED TO PROVIDE OR DO WE HAVE TO PROVIDE COPIES OF PASSPORTS ETC?!
2. DO I NEED TO SUBMIT PHOTOGRAPHS WITH THE APPLICATION?
3. I did read in the document that the biometric fee and the application fee can be combined with one check which will be worth of $385.
CAN I SEND A PERSONAL CHECK FOR THE FEE OR SHOULD I TAKE A CASHIER'S CHECK OR MONEY ORDER, IPREFER PERSONAL CHECK THAT WAY I CAN CHECK ON-LINE AND SEE IF THE MONEY HAS BEEN TAKEN BY BCIS?!?
4. WHAT DO I HAVE TO INCLUDE IF I CHOOSE TO EXPEDITE MY FORM?In a nutshell please let me know what needs to be sent with the form in the package. Thanks again for your response in advance.
