On TN and applying for i485

Hi all,

I'm currently on my 3rd TN, which is valid until October 09.
I got married and my husband is a citizen, therefore will be applying for I-485 and have a couple of questions:

1. Should I submit I-130 + I-131 + I-765 + I-485 together?
2. Does anybody know how long it takes to get the AP approved? I need to leave the country on 07/19!
3. Does everyone have to submit a medical examination or does USCIS tell you if you need one?
4. When filing I-485, are we exempt from fees for I-765 + AP? I've found contradicting info.
5. and last but not least... do any of you have a number where a real person answers the phone at USCIS for questions?
