I-131 Reentry permit Questions - Urgent

This is the second time my mon is applying for the reentry permit. She had her first one approved about two and half years ago. My questios are
1. Does she need include her previously issued permt with her new application?
2. What can she put in as the Purpose of the Trip? Is Personal Health Condition ok?
3. What's the "Class of Admission" is? This seems new.
4. Has there anyone applied multiple times for the reentry permit? Is there possibilities that the application will be rejected?

Any advise will be greatly appreciate it.

Not required , but will not hurt if you include a copy

High possibility of delay (months and months) or denial.
