Forgot to file AR-11 and now need to file I-751...

As I am looking over the papers I'm about to send in to file the I-751 I noticed I was suppose to send in some change of address form some 16 months ago when I moved... I was totally not aware of this and never filed it...

Is this a problem? or can I just go about my my application in a regular manner?

(I moved from NY to CA a few months after receiving conditional GC..)

As far as their concerned could it have been "lost in the mail"?

Finally, for the I-751, would the following suffice: joint taxes, copy of our sons birth certificate (he was already born prior to our first interview), copy of phone bill and a picture? perhaps on of our checks displaying both our names?!

WAs advised by lawyer that it doesn't hurt to file the AR-11 together with my 751 now (even though it's late)

I'm filing for change of address, do I need to fill out date of entry to USA?? (I visited Canada 2 months ago)

If it is asking for date and place of last entry, then report it.
