Name Change/Spelling difference in Brith Certificate and Passport

Hi,
My wife's Birth Certificate has her name spelt in a slightly different fashion than her passport. When she was a child in India, they made the official spelling change in the school records and also have a "gazette" (Indian document version) that makes the spelling change official.

I think this might be a problem when she applies for her Green Card. So I think I need to get some sort of Affidavit. But the sample Affidavits in this website are only for people who have NO birth certificate. What I am looking for is :
1. What is the format to get an affidavit from India for a spelling change/difference in Birth Certificate Versus Passport
2. Who should be the one writing this affidavit ? Her parents ? Or some Uncle ? Or some lawyer who knows them ?

Appreciate the help,
Thanks,
Narayan

How about you include this gazette as an addendum with a certified translation? And of course you indicate the alternate spelling in the appropriate places on the forms?

Thanks for the reply,
So you are hinting that no official afidavit is required. Just translation of gazzette.
Okay...
Anybody else done this before ? How do you think a official affidavit should look ?
Again, should that come from the parents ?
Thanks,
Narayan
