AR-11 Port of Entry?

1) What should the port of entry be when filling out the AR-11 form? I entered the US many times before through different POEs. Should I use the latest one?

2) I will move from address #1 to a temporary address #2 for 1-2 months and then move again to a more permanent address #3. Will I have to fill out an AR-11 every time I move?

3) If so, will my AR-11's look like this:

1st move AR-11
Present address: Address #2 (temporary)
Last address: Address #1

2nd move AR-11
Present address: Address #3 (permanent)
Last address: Address #2 (or should I use Address #1, since it was permanent?)
