Please help me with filling out form I-485!

Hi All,
Sorry if this has been covered, I did not come up with any thing on the search.
I've been out out status for about a year - my F1 expired in 2005, and I worked on opt for a year.

1) I'm getting ready to file for AOS through my wife. On the I-485, it asks: "current USCIS status" , do I put "out of Status" or F-1??

2) What do I put for "Expires On"? My I-94 says D/S. Do I put my F-1 visa expiration date or I-94 d/s or OPT (EAD) expiration date?

Please let me know, I would really appreciate it.

Just type D/S in "Search this Forum", and you'll find planty of information on this.
