How to change wife's name on green card

My wife was still using her maiden name when she got her green card as my spouse...Now we want to change her name..how do we do that ?

According to the letter I just got with my greencard yesterday it says:

Permanent Resident Card: If your card is lost, stolen or becomes damaged, or your name or other information shown on your card changes, use FORM I-90 to apply for a new card.

The phone number for the National Customer Service Center is 1-800-375-5283 and the web site address is: http://uscis.gov/graphics/index.htm to find forms.
The web address to download the form you are looking for is:
http://uscis.gov/graphics/formsfee/forms/i-90.htm

I concur with Laicey...also make sure you include NOTARIZED marriage certificate with the I-90 forms.

Also make sure you change the passport first.

Technically speaking your passport determines your name, once you change it in the passport it becomes a formality
