According to the letter I just got with my greencard yesterday it says:
Permanent Resident Card: If your card is lost, stolen or becomes damaged, or your name or other information shown on your card changes, use FORM I-90 to apply for a new card. The phone number for the National Customer Service Center is 1-800-375-5283 and the web site address is: http://uscis.gov/graphics/index.htm to find forms. The web address to download the form you are looking for is: http://uscis.gov/graphics/formsfee/forms/i-90.htm Hope this helps.

I concur with Laicey...also make sure you include NOTARIZED marriage certificate with the I-90 forms.

Also make sure you change the passport first.
Technically speaking your passport determines your name, once you change it in the passport it becomes a formality
