    parents' names on the birth certificate + Tax Documents

    Hi,

    Birth Certificates are required for the CP interview.
    In my case, the birth certificate shows my father's and my mother's full names (that is : first_name middle_name surname)

    My wife's birth certificate shows her fathers "first_name surname" and her mother's "first_initial middle_initial surname"

    Is that okay OR I need to get a new birth cetificate for my wife ?

    Also, what tax related documents do I take ?

    I plan to take the photocopies of the Form 1040 that I have filed for the last 3 years.

    Do I need to take any document from the local IRS office here in USA ?

    Answer: Get two affidavits from your wife's parents, elder siblings, wife's relatives who are old enough to know that she was born to her parents. I have the same case as your wife and my attorney recommended getting affidavits which i got from my mother, father and eldest brother (2 required but i got 3)!

    Also, what tax related documents do I take ? Do I need to take any document from the local IRS office here in USA ?

    * Photocopies of your last 3 years tax returns for fed, stat and city if any.
    * W-2 for same period
    * Request transcript from INS for same period by calling 800-829-1040. You will get this 3 year trancript within 10-12 days of placing the order.

    Format of the affidavit for birth certificate ?

    thanks a lot for the reply!! I think I will need to get Affidavits done from my wife's parents/relatives regarding her birth certificate.

    Can you tell us the format of the affidavit ? Are there any websites/ links where one can get such affidavit formats ?

    Also, what kind of stamp-paper ( how many Rupees?) should be used; any idea ?

    Did the interviewer ask you to show the affidavits when he/she saw that your parents' full names were not there in the original birth certificate ?

    Answer: Attached is a sample affidavit of birth that I used.

    Answer: Rs 20 per affidavit.

    Did the interviewer ask you to show the affidavits when he/she saw that your parents' full names were not there in the original birth certificate ?
    Answer: I have my interview scheduled next month. Since my attorney has advised I got affidavits from both my parents and additional one from my eldest brother!

    Once you have completed the sample affidavit show it to your attorneys and get their blessings before having your folks in india execute it. The reason I am saying this is bcoz a friend of mine got them executed in india and then lawyers took a look at it and thought the information in affdavit was insufficient and he had to redo the affidavits which i am sure caused inconvenience to his folks back home.

    So once you have typed out your information in the sampel affidavit run it by your lawyers!
