B-2 Extension

Help Needed,
For B-2 Visa extension for my mother:
1. Do we need to turn in the orginal I-94 or a copy
2. Can I write to the INS give the reason for her extension, financial support etc. (addressed from me and not my mother). Also consequently do I need to fill out part 6 "signature of person preparing other than the applicant"?
