You have to provide police clearance to the US embassy for each country:

    - where you have spent six or more than six months, and you were s16 or more than 16 years old when you visited that country.

    - You don't have to get police clearance form Bahrain because you were not 16 at that time. Also, you have not mentioned how long you had been in Bahrain.
    - You have to get clearance from Libya if you spent six or more than six years there.
    - If you were 16 years old when you visited US, then you have to get clearance from US as well.

    Best of luck!

i lived in Bahrain for 4 year's. in Libya i was more than 16s and i lived there for 3s but it's impossible to get police record's from libya because of revolution over there and i am not living there anymore neither Bahrain nor Libya . and for the USA i visited it only for 6 month's

    Thanks

Perhaps you shouldn't say that you lived in Libya.

I think it would be best for you to contact the nearest US consulate in your city. If there is no US consulate in your city, you should contact the US Embassy then.

I think that for the US you don't need to provide one. They'll check themselves.

If you are sure you are a 2011 DV winner (your case number is like 2011xxxxxxx)
