DS 230 Question for Immigrant Visa for Wife in India

If wife kept the maiden name after marriage i.e. Family name in Q1 is same as Maiden name(last name before marriage)
then do we need fill maiden name again in Q2 other names used or Alias(if married woman, give maiden name) or type None

e.g. Smith, Jane Macdonald (Family, First Middle) in Q1

then do we type "Smith" again in Q2 or type "none".

Had used Smith as last name for I-130 petition. Passport, Birth and Marriage Certificate all have Jane MacDonald Smith.

Please advice.

The beneficiary(applicant) is currently in college (graduating soon) and living in college hostel since last 4 years and goes home on weekends etc. but home(residence) in a different city but same state.
How to answer Q:30 on DS 230
Q: 30 List all places you have lived for at six months since reaching age 16

City 1 (where the hostel is) 2009 - Present
City 2 (Where her house is) Birth - Present

Is this ok?
or no need to write City 1 (where the college hostel is)
