I already got the letter from the link you provided.. The link had AC21 letter and Employment letter but not the combined one..

Malludude said, he had combined the letters, so thought i would ask him and after I get them, thought of putting it in the Dedicated link...

The letter has to be on the Employer's letterhead, of course.
