When applying as a family, Once you complete application, and complete Part B, and include all the attachments, Take one copy of everything and include it with the application (including the copy of the money order), attach real photos to the copy though.

I've applied for myself, and my wife separately, and my kids separately - my parents who I put down as relatives in India were never contacted.

If you leave India to apply for a new visa, there is the issue of needing re-entry permit if you return to India within two months on a tourist visa.

The US Officers do not actively look for stamps from other countries. On arrival US citizens or green card holders may not get an stamp on your passport

Not true. Minor's can apply on their own if they are eligible. Minors whose both parents are citizens of India are not eligible (At least one parent has to be non Indian passport holder)
You can apply for your daughter's OCI through travisa. First you will need to apply and receive your daughter's US Passport before you can apply for OCI.
The photo was there yesterday, but is it gone now.

Back to the original question.
You do not become US Citizen when your application is approved. You become a US Citizen when you take the oath. Untill after you take the oath, do not identify yourself as US Citizen on any paperwork.
