Filing for a parent concurrently,about vaccinations

Hi.
Moy mom is 76 years old and she is going to visit us soon. We are thinking about filling out forms for her for Green Card. My question is: does she need to get police report from the country she lives before she comes here if on the time we send all papers to USCIS she will be here for almost 6 months? Does she need to bring with her records about all vaccinations from childhood till now, only her age appropriate vaccines records or she doesn't need it?

Police report is not needed for AOS. Vaccine records are relevant for the medical they do with the civil surgeon; if there are no records then the civil surgeon might want them to do the vaccinations again; it's up to the specific civil surgeon.
